Community Health Network announces Community Collaboration for Health Equity Grant program

Community Health Network recently completed a Community Health Needs Assessment (CHNA) with corresponding Implementation Strategies. The CHNA identifies the significant health needs our community is facing and the Implementation Strategies define how CHNw will work to address the identified needs over the next three years.

We are excited to partner alongside local, community-based organizations to improve health outcomes and health equity for our neighbors in Central Indiana. Likewise, we're proud to extend this funding opportunity and look forward to the opportunity to partner with you!

The Community Health Needs Assessment identified five significant health needs.

  • Mental Health & Access to Mental Health Services

  • Substance Use Disorders & Overdose

  • Obesity & Chronic Disease

  • Maternal, Infant & Child Health

  • Social Determinants of Health (transportation, food insecurity, housing, poverty, access to care)

We are looking to support organizations who are addressing one or more of these health needs:
